Your Right to Information From Your Health Care Provider Applies to all health plans. If you request it, a HCP must give you information about her/his qualifications, training and experience. A health plan cannot punish or forbid a HCP from: - Fully informing you or your representative of all the treatments, therapies, consultations or tests that apply to your condition or disease, even if your plan does not cover them. - Explaining how the plan’s requirements and limitations affect you. - Advocating on your behalf. - Reporting to the State a plan’s practices that the HCP believes has affected the quality of health care.
